New Patient Registrations Welcome!

Everyone in the UK has the right to free care from a GP.  If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.

Our practice boundary is shown on the map.

Please click on the map and enter your postcode into the search box at the top of the screen. Your house will then be identified by a small red icon, if the icon is inside the blue boundary we will be able to accept your registration.

Out of area patients

If you live outside of the boundary shown by the blue line, you may still be able to register as a patient – please read our practice policy on out of area registrations here: Out of Area Registration Policy

Please note that out of area patients may not be eligible for certain services. These services include:

  • Home visits: If you are too ill to attend the surgery in the future, we will not be able to offer home visits.
  • Local/Community services: You may not have access to some community services provided in your local area such as community nursing, midwifery, physiotherapy or similar services.

Please read the following information before registering

Please ensure that you carefully read the registration form and fill in as much information as possible, including your present and previous addresses, details of your previous GP (name and address) and your NHS number if known. This will ensure a smooth registration without any delays. You may be contacted by a member of the practice if they require further information or clarification.

Photographic ID may be requested if you require online access e.g. (passport, driving license, etc.).

It is recommended that all new patients who take regular medication or are over the age of 40 years old have a health check with our Health Care Assistant.

Please note that face to face appointments may be offered at either of our two sites, Beeston Hill & Parkside Surgery, depending on availability.
