Dear Patient,

Hypertension Annual Review 1st Reminder

Your hypertension annual review is now due. The purpose of this check is to make sure that your blood pressure is well controlled and also that you are not suffering any side-effects from your medication.

Hypertension is a major contributor to the development of cardiovascular disease and affects more than 25% of the UK population. The prevalence increases with age such that more than half of all people older than 65 years have a BP of >140/90mmHg. The attributable risk of stroke due to hypertension is >60% and that of coronary artery disease approximately 40-50%. However treating and monitoring hypertension with annual reviews can prevent or postpone cardiovascular events, which is why we would like you to attend for a review.

If you do not already have an appointment for a hypertension review with a HCA, please can you contact the Patient Care Team to book your annual review.

If you do not attend for your review the GP may decide to stop prescribing your medications, as it may not be safe to continue doing so without the recommended monitoring reviews.
